It’s not really possible to define if an accident will occur. But based on experience of what has happened in the past it is possible to get an idea of what types of accident might occur and whether it is comparatively likely to happen or not. Thankfully in the radiation world there are a lot of safety requirements so accidents are pretty rare, and included in the safety requirements is the need to be prepared to react quickly in the rare event that an accident did occur.
